Reporting to the Care Manager, you will use your strong assessment, organizational, and leadership skills to deliver quality nursing care to clients in the community and to provide leadership and supervision of Health Care Aides. The successful candidate will play a key role on the interdisciplinary team, collaborating in the delivery of holistic, client and family centered care. Your role will require you to work independently in client homes, lift and carry medical bags and be comfortable working in all weather conditions. In this role, you will utilize your strong oral and written communication, and deliver quality nursing care to clients in all developmental stages in the community. Utilizing your experience in wound care, palliative care and client teaching, you will take a collaborative approach to meeting health goals, contribute to the nursing diagnosis and participate in nursing interventions and outcomes. Transferring patients of varying sizes and degrees of mobility. Transporting supplies and equipment. Driving in all weather conditions. Preferred Qualifications: Minimum of one year Home Care experience. Minimum of one-year Acute Care experience. Experience with wound care, palliative care and client teaching.

The LPN delivers quality nursing care to community clients and provides leadership and supervision to Health Care Aides. They collaborate within an interdisciplinary team to provide holistic, family-centered care in client homes.

Candidates must have a practical nurse education program completion, a valid CLHA practice permit, and BCLS-HCP certification. A valid Class 5 Driver's License and the ability to meet physical demands including lifting and driving are required.
