POSITION SUMMARY We are seeking a compassionate, collaborative, and patient-centered Addiction Medicine Physician to join our interdisciplinary team. In this role, you will provide assessment, treatment, and ongoing management for individuals experiencing substance use disorders and concurrent mental health conditions. You will play a key role in delivering evidence-based care, advancing harm reduction approaches, and supporting integrated, community-based services. KEY RESPONSIBILITIES Conduct comprehensive addiction medicine assessments and develop individualized treatment plans. Provide evidence-based care, including medication-assisted treatment where appropriate. Monitor patient progress and adjust treatment plans to support recovery and wellness goals. Collaborate with interdisciplinary team members, community organizations, and healthcare partners to coordinate care and improve patient outcomes. Maintain accurate, timely, and thorough clinical documentation. Participate in program development, quality improvement initiatives, and service enhancement activities. QUALIFICATIONS Licensed or eligible for licensure with the College of Physicians and Surgeons of Ontario (CPSO). Certification in Addiction Medicine or equivalent clinical experience is considered an asset. Experience providing care to individuals with substance use disorders and concurrent mental health conditions. Demonstrated commitment to harm reduction, patient-centered care, trauma-informed practice, and health equity. Strong communication, collaboration, and relationship-building skills. PREFERRED EXPERIENCE Ability to provide regular in-person clinical services; virtual care may be available in certain circumstances. Experience working collaboratively with community organizations, municipal and social service partners, primary care providers, and other healthcare professionals to improve access to addiction services. Demonstrated success in fostering partnerships that support integrated, coordinated, and community-based models of care. The physician will conduct comprehensive addiction assessments and develop individualized treatment plans using evidence-based care and medication-assisted treatment. They will collaborate with interdisciplinary teams and community partners to coordinate care and improve patient outcomes.

Candidates must be licensed or eligible for licensure with the College of Physicians and Surgeons of Ontario. Experience with substance use disorders, concurrent mental health conditions, and a commitment to harm reduction are required.
