Specific Accountabilities:

Centralized Intake and Needs Assessment:

  • Be the first point of contact for students needing integrated support services

  • Conduct intake assessments to identify academic, personal, financial, accessibility, wellness, and socio-economic barriers impacting student success

  • Gather relevant documentation and student information, including demographic data, disability disclosures, medical conditions, presenting concerns, and support needs

Integrated Care Coordination:

  • Develop integrated support plans based on student needs and identified barriers

  • Facilitate “warm referrals” that include detailed documentation and relevant student information to internal college services

  • Follow-up on referrals and monitor service engagement Case Documentation and Service Continuity

  • Maintain accurate, confidential, and timely records of student intake, referrals, service connections, and outcomes

  • Use appropriate documentation practices to support integrated service delivery and continuity of care across service areas

  • Assess and gather student related information as it pertains to their support needs, such as appropriate medical and psychological documentation, as well as psychoeducational assessments and previous Individualized Educational Plans

  • Track referral patterns and student support needs to identify service gaps and opportunities for enhancement

  • Support coordinated case reviews and consultations with internal service areas and external agencies where necessary

Student Advocacy and Equity Support:

  • Advocate for student needs and promote timely access to integrated supports
  • Foster equitable access to services for students from equity deserving populations, including First-Generation, Indigenous students, Black and racialized students, LGBTQ+ students, International students, students with disabilities, and students experiencing socioeconomic barriers
  • Create an inclusive and supportive intake experience that minimizes re-traumatization and fosters student dignity

About the Position:

This is a funded PNRK (Project of a Non-Recurring Kind) part-time support staff Bargaining Unit role. The successful incumbent will report to the Associate Director, Student Success & Access Programs working 35 hours per week at an hourly rate of $38.61. The length of the role is 1 year.

The Integrated Intake and Care Coordinator will serve as the primary intake point for students needing coordinated supports and services across the college community. The incumbent will be responsible for intake assessment, triage, referral coordination, and case documentation to ensure students receive seamless and integrated support without systemic barriers, unnecessary duplication, or the retelling of a student’s narrative.

The incumbent will support students experiencing barriers related to disability, mental health, financial, housing, and food insecurity, cultural barriers, systemic inequities, and other complex challenges. Adopting a centralized intake process, the Integrated Intake and Care Coordinator will ensure students are connected to the appropriate internal and external services through coordinated referrals and integrated documentation practices.

Qualifications:

  • Minimum 2 year post-secondary diploma in Social Work, Child & Youth, Human Services, Disability Studies, or Indigenous Studies
  • Minimum of 3 years’ experience supporting clients or students with intake, assessments, triaging, and developing integrated support plans
