You will be a welcomed member team and impact in the following ways: * Establish, maintain, and communicate current resident assessment schedule to inter-professional team. * Ensure RAI accuracy and compliance with coding, capping and care plans are updated. * Review and analyze each resident’s RAI assessments to ensure accuracy and meet the residents needs. * Monitor and analyzes RAI outputs and reports. * Produce RAI reports for leadership use to assist in clinical and operational decision-making * Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC). Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization. * Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees. * Lead the RAI education process for all participating team members Who You Are: · A Registered Practical Nurse in good standing with the Ontario College of Nurses * Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC * RAI experience/education is an asset * Good problem solving and analytical skills What you need to know: * Sienna Senior Living is committed to creating an inclusive environment where all team members and residents feel like they belong.
